Claims submitted by mail are considered filed on the date postmarked within the United States. Claims submitted outside of these time frames will be denied as untimely. Patients may not be charged more than the 20% coinsurance and deductible (if applicable) on claims that have been submitted untimely. These established limits for filing can be waived if it can be documented that the claimant failed to file within the time limitation because of an administrative error or delay on the part of the Social Security  Administration (SSA) or the Medicare contractor.
Please find here the new updated filing limit for Medicare.
Late Filing Reduction
Assigned claims for services within the time limits but submitted after one year from the date of service will be reduced by 10%.
