Resubmit the claims with Medical documents.
M34 Claim lacks the CLIA certification number.
Resubmit the claim with CLIA number.
M52 Missing/incomplete/invalid “from” date(s) of service.
M53 Missing/incomplete/invalid days or units of service.
Check the DOS.
