Missing/incomplete/invalid number of coinsurance days during the billing period.

Missing/incomplete/invalid number of lifetime reserve days.

Missing/incomplete/invalid patient name.

Check the patient name

Missing/incomplete/invalid patient's address.

Check the patient addrress.


MA39
Missing/incomplete/invalid gender.

Change the gender and rebill.


MA40
Missing/incomplete/invalid admission date.

All the hospital claims require admission date. Check it.



MA43
Missing/incomplete/invalid patient status.

Status is missing like student or employed.
